Job Description:

Complete all required duties of care management with parents as partners in a 24/7 environment. Build and lead the Child Family Team process and facilitate the Team’s negotiations toward self-sufficiency. Coordinate & facilitate comprehensive assessments of strengths & needs of children & families in their community. Facilitate the development of ISP’s (Individual Service Plans) for children and their families that allows and respects individual choice and is outcome-based. Develop transition plans for the child & their family out of CMO & into community-based services. Maintain accurate, timely and comprehensive records for each child & family. Coordinate care management services with system partners. Coordinate service information with the Community Resource Manager and prepare required reports as prescribed by Quality Management System. Supervisor Experience:

Care Management:

1 year post graduate experience (Required) Working with children at risk: 1 year (Required)

Education:

BA/BS degree in a relevant field (e.g. Social work, counseling, psychology, special education, mental health field) Bilingual (able to speak, read and write in Spanish and English) strongly desired
