Social Work Supervisor Position Available In Middlesex, New Jersey

Social Work Supervisor 2nd Home Adult Medical Day Care Perth Amboy 420 Fayette Street, Perth Amboy, NJ 08861 2nd Home Adult Medical Day Care Center is looking for compassionate and dedicated Certified Social Worker to join our team!

• Coordinates discharge planning. .Maintains quarterly reports on time. Maintains a written record of the frequency and nature of the social service consultation and services provided to participants.
• Ensures that participants receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights.
• Protect participants from abuse, and cooperate with all investigations.

Report any allegations of abuse, neglect, exploitation, or mistreatment of participants to supervisor and/or administrator.

Job Requirements:

Bachelor’s degree in social work. CSW certification required. Bilingual Spanish Preferred. Excellent interpersonal skills and knowledge of all governmental regulations relating to the social services department. Ability to work independently, problem solve and makes decisions as necessary. Ability to interact fluently with government agencies and community based agencies.
