Job Description
Social Worker, Level III:
New York City Health and Hospitals Corporation (Bronx, NY) seeks Social Worker, Level III to provide mental health evaluation, treatment, and coordination of aftercare services to adults and children residing in Domestic Violence Shelters and via Tele-Health. Conduct Social WorkComprehensive Assessments for all patients in accordance with departmental policies and procedures. Participate in case conferences and team meetings to review progress and status of all patients. Provide supportive counseling, crisis intervention, therapy, grief counseling and advocacy and referral services to shelter clients and children. Lead psychoeducational/support group for adults, children and adolescents. Assist patients with adjustments to treatment, including problems of depression, anxiety, denial and/or anger, and noncompliance with medical regimen. Formulate and implement social work treatment/continuing care plans. Collaborate with domestic violence shelter staff to ensure comprehensive delivery of psychiatric services to clients as needed. Confer and consult with professionals in other disciplines to determine and implement a multi-disciplinary approach to patient care and to develop treatment plans, evaluate patients’ progress, develop discharge and aftercare plans, and recommend alternative action or treatment. Provide outreach services, makes referrals to other governmental, public and/or community agencies, prepare patients to accept those services, provide direct social work services on high-risk cases and escort patients in connection with community activities and referrals, as needed. Assist in plans for institutional or hospital discharge and other post institutional/hospital care; evaluate alternate care arrangements for patients, where needed. As a mandated reporter, document and report suspected cases of child abuse or neglect to the New York State Central Registry. Appear and testify at administrative and court hearings, as necessary. Responsible for monitoring all patients in case load from referral to intake to post-referral treatment to ensure good patient care. Ensure patient records are stored securely, confidentially, and compliant with HIPAA standards. Participate in and assist in the planning of community events including health fairs and domestic violence/sexual assault awareness month activities. Requires frequent travel to domestic violence shelters located within New York City.
Requirements:
Master’s degree in Social Work or a closely related field. Candidate must be a Licensed Clinical Social Worker (LCSW) in the State of New York. Candidate must have completed coursework in Child/Adolescent Trauma, Grief/Loss/Bereavement, and Dialectical Behavior Therapy. Requires three (3) months of experience providing direct care and service to patients who experienced trauma.
