Position Summary:
The social worker identifies and addresses the presenting needs and challenges of the children and families served in our education program. They are responsible for providing individual and group counseling as indicated on the students’ Individual Education Plan (IEP). The social worker also provides clinical consultation to team members and assists staff in working effectively with the children and families. The social worker acts as the facilitator for planning meetings from the point of admission through discharge. They maintain open communication with all service providers and families involved. This position uses a multidisciplinary team approach.
Responsibilities:
Identify and assess the strengths and challenges of students and families and provide individual group and family counseling to address needs and support enhanced academic and behavioral performance in school. Provide crisis interventions, planning and strategies. Complete numerous assessments on intake and monitor periodically. Develop treatment plan goals and objectives based on assessments; Monitor progress according to OMH regulations. Develop and regularly review Individual Crisis Support Plans for all students on your caseload. Schedule and facilitate periodic Treatment Team meetings, according to OMH regulations. Participate in Psychiatry appointments with students on your caseload.
Requirements Education:
Master’s Degree in Social Work, Psychology or related field required . Limited Permit, LMSW, LCSW, LMHC, LMFT, Licensed Psychologist, or similar required Supervision available for candidates operating with a
