Job Description
Director of Social Work Skilled Nursing Care of Dutchess County Beacon, NY 12508 Skilled Nursing Facility located in Dutchess County, New York is seeking to hire a Director of Social Work (BSW, MSW, LMSW) to work in our facility. The ideal candidate will be a professional, compassionate and outgoing individual with extensive experience in a skilled nursing/long-term care setting. The Social Worker will plan, develop, organize, implement, evaluate, and complete all social service requirements in accordance with current existing federal, state, and local standards (as well as established facility policies and procedures), to assure that the medically related emotional and social needs of the residents are met and maintained on an individual client basis.
Requirements:
Knowledge of age specific developmental factors specific to adult and geriatric residents (i.e., physical, cognitive, and socialization factors) in directing the overall operation of the facility’s delivery of services. Long term care/ sub-acute experience required Performs resident evaluations as required by state regulations. Support state regulations for admission and retention of residents. Assists residents and families with the transition process. Orients residents and families to the facility. Assists and interacts with residents and families to access services. Knowledge of discharges planning and assistance that address the identified needs of adult populations. Participates in development and implementation of each resident’s individual care plan. Communicates with other members of the management team to ensure coordination of services to residents. Documents case management notes in residents’ files as necessary. May perform other duties as assigned. Knowledge of MDS and discharge planning are essential.
Qualifications:
BSW or Master’s Degree in a related field required. Experience in the field of social work providing services to an adult population as required by state regulation if applicable. Two to five years experience in social work providing services to an adult population is preferred. Basic computer knowledge, particularly in Microsoft Word and Excel. Certified/Licensed as required by state regulations if applicable. Valid driver’s license.
