Social Workers are needed for an outpatient/community health organization in Brooklyn, NY. Prior experience in mental health, substance abuse, or co-occurring disorders preferred. High integrity and character are a must. —- We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) to provide comprehensive mental health and substance use support to individuals facing emotional, behavioral, and addiction-related challenges. This role involves assessing, counseling, and connecting clients with essential resources to promote stability and long-term recovery. The ideal candidate has at least two years of experience working with individuals affected by mental health disorders, substance abuse, or co-occurring conditions. This position requires a strong understanding of trauma-informed care, harm reduction strategies, and evidence-based interventions to empower clients on their path to recovery.

Key Responsibilities:

Conduct comprehensive biopsychosocial assessments to develop individualized treatment plans. Provide individual and group counseling focused on mental health and substance use recovery. Facilitate crisis intervention and develop risk management plans for at-risk individuals. Collaborate with psychiatrists, therapists, case managers, and community resources to ensure coordinated care. Utilize evidence-based interventions such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and Harm Reduction approaches. Support clients in navigating housing, employment, financial assistance, and healthcare services. Provide psychoeducation and relapse prevention strategies to individuals and families. Maintain accurate documentation in compliance with agency, state, and federal regulations. Advocate for client needs within healthcare systems, legal settings, and social service agencies.

Requirements:

LCSW or LMSW in the state of NY Minimum of 2 years of experience in mental health, substance abuse, or co-occurring disorders. Knowledge of trauma-informed care, harm reduction, and recovery-oriented approaches. Experience in crisis intervention, case management, and care coordination. Strong interpersonal and communication skills, with the ability to build rapport with diverse populations.
