PUPROSE OF POSITION
To provide and collaborate with internal and external sources to provide comprehensive services to homeless safety net patients.

SUMMARY OF DUTIES AND RESPONSIBILITIES
Under the supervision of the Director for
Social Work Services, Associate Director, or Social Worker Level V:
- Complete biopsychosocial per referral and assessment policy. Which includes holistic history of homelessness through a review of EPIC and other relevant data bases. Plan and carry out appropriate interventions reflective of trauma informed principle.
- Provide ongoing supportive counseling to patients, families/ significant others as well as concrete services.
- Develops and implements treatment plan appropriate to patient’s needs based on the assessment.
- Ability to understand and learn about the landscape of homelessness in New York City metro area, including but not limited to the Department of Homeless Services (DHS) shelter system, migrant shelter system, street outreach landscape, housing landscape, and outpatient behavioral health services such as Assertive Community Treatment (ACT)/Intensive Mobile Treatment (IMT) teams.
- Coordinate with the housing navigation team by a consistent review of the General Housing Intake Tracker (GHIT), monitoring patient referrals and progress, submitting supportive housing 2010E applications, and scheduling conferences as needed.
- Completes referrals to Crisis Respite, Medical Respite, Joint Command Center (JCC) request, Single Point of Access (SPOA) applications, Safe Options Support (SOS) referrals, and others, when appropriate.
- Coordinates with patients’ external care teams, such as ACT/IMT teams, shelter case management teams, SOS teams, etc., when appropriate.
- Collaborates with inpatient teams in care planning and transitional care for patients attributed to the Safety Net Clinic or Street Outreach and Wellness (SHOW) panel.
- Collaborates with team members such as the SHOW Social Worker and Safety Net Clinic Community Health Worker (CHW) teams.
- Attends Social Work Meetings, individual, unit and departmental hospital meetings as mandated by the departmental administrator.
- Participates in supervisory conference with social work supervisor.
- Provide supervision and mentoring support to assigned social work staff. Activities may include onboarding new hires, lead training sessions, review documentation and serve as a resource for difficult or complex care coordination issues.
- Completes performance appraisals for assigned social work staff.
- Completes required documentation in a timely manner as per departmental and hospital policies.
- Participates in the Department’s cross training/ cross coverage program.
Minimum Qualifications For Assignment to Level III
