$24.22•$33.91 The referenced pay range represents the minimum and maximumpensation for this job. Individual annual salaries/hourly rates will be set within the job’spensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:
GENERAL PURPOSE

Under the supervision of a licensed social work supervisor but with reasonable independence, the Alzheimer’s and Dementia Care Program BSW provides consultation and social work services to Alzheimer’s and Dementia Care Program patients and their families. Responsibilities include intake, outreach, supportive and educational case management services to patients and families in their home setting as well as within the ambulatory clinic, served by the Alzheimer’s and Dementia Care Program and Memory Care Program. The BSW collaborates with the Memory Care Program team, mostmonly the medical director and the practice NP’s, to assist patients who have identified social work needs. The BSW also provides consultation to the health care providers regarding patients’ care needs and/ormunity resources.

ESSENTIAL FUNCTIONS

Participates in Social Work Division and Alzheimer’s and Dementia Care Program/Memory Care meetings as indicated. Participates in social work supervision/consultation about his/her work in accordance with plan development with supervisor and practice site MD Directors. Participates in required activities of the Social Work Division and Alzheimer’s and Dementia Care/Memory Care including, but not limited to, timelypletion of statistical reports/forms, time reporting and other Memory Care or Division forms, training programs and University Health Service reviews. Participates in assignedmittees. Provides social work services and/or consultation in collaboration with NPs for patients with problems identified through psychosocial assessments. Services may include crisis intervention; problem-solving; information and referral, respite care planning; end of life support; and assistance with environmental, transportation and/or legal concerns. Services are largely provided within the patient/family home or by phone. Collaborates with other practice care providers in development and implementation of interdisciplinary care plans. Coordinates referrals to and acts as liaison tomunity agencies regarding services to individual patients in collaboration with medical director and practice NPs. Provides educational support to patients and caregivers and maintains current knowledge ofmunity resources relevant to the Memory Care patient population. Documents all social work activities in the patients’ medical record in accordance with the relevant Social Work Division and Alzheimer’s and Dementia Care/Memory Care practice policies and procedures. model. Participates in relevant training and educational opportunities designed to enhance knowledge and skills. Provides social work consultation to other care providers regarding patient problems andmunity services. Participates in regular supervision, both scheduled and on an as-needed basis. The BSW is accountable administratively and clinically to his/her social work supervisor and through him/her to the Director of Social Work and Patient & Family Services for the provision of social work services and adnce to Social Work and ID policies, procedures and standards of professional conduct. The BSW’s job performance must conform to the expectations set forth in the position description. In addition, the BSW is accountable to Alzheimer’s and Dementia Care/Memory Care Program practice site Medical Director for providing social work services in accordance with policies and standards established by Alzheimer’s and Dementia/Memory Care. Other duties as assigned by supervisor.

MINIMUM EDUCATION & EXPERIENCE

The social worker must have a BSW from an accredited social work program. 1-2 years’ experience in a clinical or home care setting (some ambulatory care experience preferred).

KNOWLEDGE, SKILLS AND ABILITIES

Excellentmunication and team skills including ability to form strong collaborative interdisciplinary partnerships across care settings. Soundputer knowledge and skills including an aptitude for using health information technology to guide care management activities and functions. Ability to work independently and meet deadlines. Creativity and stronganizational skills.

LICENSES AND CERTIFICATIONS

Valid NYS driver’s license including access to reliable transportation that enables fulfillment of position’s travel requirements.
