is headquartered on the Upper West Side of Manhattan and the team works on a hybrid schedule. The Assessment Social Worker will be expected to work in the office 3 days per week and as business needs require. All staff members are required to be fully vaccinated for

ABOUT THE CONSTITUENT & COMMUNITY SERVICES

(CCS)

DEPARTMENT

The CCS department provides a range of supportive services that assist older adults in aging in place and connecting to programs and resources that help them thrive and build vital social connections. The department provides support to a broad spectrum of older adults who range in age from 60 to over 100 and vary in physical mobility from highly active in the community to predominantly home-based. To support our social work model of service delivery, staff in CCS are consulted by colleagues throughout the agency as constituent needs arise. The robust CCS portfolio includes Case Assistance, Information & Referral, Kosher Meals at Home, Friendly Visiting, Home Assessments, and Synagogue Partnerships.

THE OPPORTUNITY DOROT

social workers provide an essential layer of expertise within our programs and contribute to DOROT’s overall success. The Assessment Social Worker has primary responsibility for conducting psycho-social home assessments of new older adult participants and follow-up assessments of ongoing participants; a critical piece of this process is to work with each individual older adult to identify supportive services that DOROT and other community partners offer to enhance their overall well-being. This position also plays an integral role in introducing older adults to DOROT’s dynamic array of programs and assessing which opportunities best meet their interests, needs, and abilities.

PRIMARY RESPONSIBILITIES

Conduct in-home assessments working independently in the community.

This includes:

Discussing program opportunities that support the older adult’s priorities and ability to meet program criteria. Providing short-term case assistance, information, referral, and advocacy to older adults and their caregivers following the assessment. Collaborating with the social work team and other staff to initiate the process for program participation(s). Collect, track, and maintain case assessment and related client interactions using DOROTs’ online database; ensure accuracy and detail of records. Facilitate or co-facilitate one or more support or psychoeducational multi-session groups each year. Serve as the assigned social worker for a caseload of 15-20 Friendly Visiting matches, conducting quarterly check-ins and ad-hoc support to participating older adults and volunteers. Provide support to and participate in DOROT’s community-wide intergenerational volunteer programs and onsite events. Staff the Information and Referral (I&R) telephone line, as needed. Special projects and additional duties as assigned. This position requires use of public transportation for occasional visits to older adults’ homes. Travel expenses are covered by

DOROT. QUALIFICATIONS

An LMSW or a Licensed Mental Health Counselor Affinity for working with an older adult population Strong interpersonal, clinical, group facilitation, and presentation skills Strong written and verbal communication skills Strong organizational skills, with an ability to work independently and collaboratively Passion and sensitivity for DOROT’s mission Knowledge of local aging and community resources a plus Experience working with volunteers a plus Availability to work approximately 8 Sundays and/or work nights per year for programs or events; this includes up to four events that may not be within your regular scope of work Proficiency with Microsoft Office Suite; experience with Salesforce or a similar constituent management system/database a plus
