Acute Care Medicine Responsibilities:

Conduct psychosocial assessments and develop individualized treatment plans Assist with hospital discharge planning and evaluate alternate care arrangements Provide counseling and concrete social services in both inpatient and NYC community settings Collaborate with medical teams to develop, implement, and evaluate treatment plans Make community referrals, assist patients in accessing services, and provide outreach Act as a mandated reporter and participate in court proceedings when necessary Document patient interactions, maintain accurate records, and prepare reports Participate in team meetings, committees, quality improvement activities, and training Ensure compliance with hospital policies, safety protocols, and regulatory standards Provide clinical services under supervision when required

Qualifications:

Active New York State dedicated Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) Prior experience in an Acute Care and/or Hospital setting Strong background in Discharge Planning and Patient Care Coordination Proficiency in Microsoft Word and Excel Excellent organizational, communication, and writing skills Ability to thrive in a fast-paced, high-volume healthcare environment
