We are seeking an NY Licensed Social Worker (LCSW) to join our client’s team.

SALARY/HOURLY RATE

$72,500 Annually

POSITION OVERVIEW

This NY Licensed Social Worker (LCSW) role involves working directly with clients across the five boroughs, providing assessments, support, and advocacy to ensure they receive the care and services they need.

RESPONSIBILITIES OF THE NY LICENSED SOCIAL WORKER

(LCSW):
Conduct client assessments and develop individualized care plans.
Provide counseling and support services to individuals and families.
Collaborate with healthcare providers, community agencies, and internal teams.
Maintain accurate and timely documentation in compliance with HIPAA.
Advocate for client needs and connect them with appropriate resources.
Participate in team meetings and contribute to a supportive work culture.
Review and approve policy and procedure manuals to ensure compliance with Article 36, Title 10 of the New York State Rules and Regulations (10NYCRR).
Conduct pre-opening tasks including document reviews, conference calls, and on-site surveys.
Write compliance letters to agencies outlining necessary revisions when manuals do not meet Article 36 standards.
Recommend licensure or denial to the Department of Health (DOH) following pre-opening surveys.
Follow up with applicants on outstanding manual corrections and required documentation.
Recommend application withdrawal to the DOH when applicants fail to submit the required materials.
Evaluate agency readiness during on-site visits, including review of environmental space, equipment, personnel files, and staff knowledge of policies.
Conduct operational surveys of Licensed Home Care Services Agencies (LHCSAs), reviewing client and personnel records, QA/QI meeting minutes, governing authority documentation, complaint logs, and more.
Prepare Statements of Deficiencies (SODs) using the DOH’s ASPEN system when applicable.
Assist in scheduling and preparing for pre-opening site visits.
Review site assessment reports and consult with the Director on findings.
Coordinate travel logistics and materials for field visits.
Maintain flexibility in the work environment while in the field.
Collaborate with co-reviewers and meet at designated locations.
Foster a team-oriented environment built on mutual respect and shared goals.
Track work progress and documentation as required by the LHCSA Director.
Support special projects and additional responsibilities as assigned.

REQUIRED EXPERIENCE/SKILLS FOR THE NY LICENSED SOCIAL WORKER

(LCSW):
Strong communication and interpersonal skills.
Ability to work independently and manage a field-based schedule.
Detail-oriented with excellent documentation skills.

PREFERRED EXPERIENCE/SKILLS FOR THE NY LICENSED SOCIAL WORKER

(LCSW):
Experience in community health, home care, or adult care settings.
Familiarity with Department of Health (DOH) programs and procedures.
Bilingual skills are a plus.

EDUCATION REQUIREMENTS

Master’s degree in Social Work from an accredited institution.
Current LCSW license in New York State.
