Social Work Case Manager This job will have the following responsibilities:
- Performing case management telephonically for members with behavioral health and substance abuse or substance use disorder needs.
- Conducts assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment.
- Monitors and evaluates effectiveness of care plan and modifies plan as needed.
- Supports member access to appropriate quality and cost effective care.
Hours:
8-5pm in associate’s time zone. Prefer candidate in CO, CA or NY.
- Manages behavioral and psychosocial needs that result in improved clinical and financial outcomes and delivers social work interventions.
- Assists members to effectively utilize available resources to meet their personal health needs and help them develop their own capabilities.
Qualifications & Requirements:
Requires MS or MA degree in social work, counseling, or a related behavioral health field. Current active unrestricted license as an
LMSW, LPC, LMFT, LMHC, LAPC, LCSW.
