Job Summary The Licensed Social Worker (LMSW) provides short-term support to older adults as they navigate their needs. Through phone consultations, the social worker assesses the individual’s concerns and urgent needs, offering both internal and external referrals to partner agencies for assistance. This role may also involve advocacy and outreach to other social service or case management programs to effectively meet the unique needs of older adults. Key Responsibilities Provide short-term support to older adults through phone consultations. Assess individual concerns and urgent needs of clients. Offer referrals to partner agencies for further assistance. Conduct advocacy and outreach to social service or case management programs. Conduct around 3 home visits per week within the same area. Manage referrals from various programs and staff, as well as the Information & Referral line.
