Full-Time Social Worker

• Manhattan Office Affirmed Home Care is growing — and we’re looking for a compassionate, motivated Licensed Social Worker (LMSW or LCSW) to join our Manhattan team full-time!

As our Social Worker, you’ll play a vital role in supporting our clients and their families with personalized care strategies, resource coordination, and emotional support — helping them thrive safely in their homes.

What You’ll Do:

Assesses clients and/or family psychosocial status, social work needs and living conditions utilizing professional knowledge, skills of observations and interviewing skills. Serves as the primary point of contact for clients and families for all non-clinical care matters; conducts and directs meetings and calls with clients and their families. Work with clients and families to evaluate their needs and connect them to appropriate services. Facilitate home visits to ensure safe and supportive environments for clients and families. Provides social work services in accordance with Affirmed Home Care policies, practices, procedures and Standards of Social Work Practice. Participates in the development of treatment plans and revises the goals as needed. Coordinates approaches to patient and/or family care with other team members. Consults with and educates the clients and families regarding the treatment plan and community resources. Looks for opportunities to enrich and support the client in the home environment, to accommodate their success in the home, and support aging in place.

What You Bring:

Master’s Degree in Social Work (MSW) Current New York State LMSW or LCSW license. At least 1 year of experience in a healthcare setting (home care experience is a plus!). Familiarity with community resources and service planning. Excellent communication, organizational, and interpersonal skills. A passion for supporting others and making a lasting difference in people’s lives.
