Director of Social Work Healthcare Nursing Center Canandaigua, NY 14424 Full Job Description New Exciting Opportunity! Healthcare Nursing Center is seeking an experienced Social Worker to join our team! This fantastic Social Worker will work in our Rehab and Skilled Nursing facilties to provide new resident’s with a meaningful and comprehensive orientation to the facility. Serving as the advocate for our residents, the Social Worker will function in the role of liaison between the resident, family members, and staff responsible for providing care and services to the resident.
EDUCATION, EXPERIENCE & QUALIFICATIONS
Must possess a Bachelor’s Degree in Social Work. Experience in Geriatrics preferred
DUTIES AND RESPONSIBILITIES
Provide new resident orientation to the nursing home. Liaison for residents and families with nursing staff. Reviews and follows up on any concerns for the residents and staff with all incident reports and investigations and reports as appropriate and necessary. Schedules resident appointments and arranges for transportation. Takes charge with discharge planning as appropriate and necessary. Works with caregivers of the nursing unit and all departments to provide the residents with a fulfilling living experience by providing a chance to develop new relationships with others, to foster personal growth and to continue to live as productive adults. Abides by the Resident’s Bill of Rights. Conducts family meetings and nursing unit meetings. Keeps supervisor informed of unusual occurrences that relate to the health and safety of residents, staff and families. Oversees care plans, MDS and quality improvement programs. Completes all tasks as assigned by Supervisor and Administration
PERSONAL REQUIREMENTS AND WORKER TRAITS
Must possess the ability to deal tactfully with personnel, residents, family members, visitors and the general public. Must be able to cope with the mental and emotional stress of the position; is subject to frequent interruptions. Must be willing to work as a team player. Must be able to relay information concerning a resident’s condition. Must be able to relate to and work with the ill, disabled, elderly and emotionally upset people within the facility.
