Position Summary:
As a Family Peer Advocate for the Sullivan County Youth ACT (Assertive Community Treatment) program, you have the opportunity to work as part of an interdisciplinary team that helps families with children ages 10-21 avoid hospitalizations and out-of-home placements by providing skills and support to youth and families to meet all of their needs and overcome their challenges. This role is based on personal experience, not educational or professional qualifications. Applicants must have raised, fostered, or had guardianship of a child involved with service providers (e.g., DSS, CPS, mental health, OPWDD, substance abuse) or facing challenges like behavioral issues, IEPs, or placement.
Responsibilities:
Ample support and supervision to work as a team to provide intensive community-based services to youth and families that the team serves Specialized training in the Youth ACT model Opportunity for growth in agency A lead role in providing services to caregivers/families such as: Empowering caregivers Helping caregivers advocate for themselves and their children Providing outreach and information Collaborating with caregivers to build skills/parenting toolkit Encouraging collaboration and a whole family approach Supporting caregivers and engaging them in their child’s care Creating a safe place and opportunities for transparency Teaching self-care and coping strategies Building natural supports and community connections
Requirements Education:
High School Diploma or GED required . NYS Family Peer Advocate credential is required within 30 days of employment.
Experience:
