Provides support and follow-up to clients in the community. The Peer Advocate will be an integral part of the HOPE Housing programs to assist individuals and families with accessing emotional, informal supports. The peer advocate will work closely with the Hope Housing Team and the clients in the community. Candidate must be flexible as work hours may include some evenings to accommodate program needs.
RESPONSIBILITIES
Provide recovery support to clients by sharing life experience. Document in Foothold EHR system the services provided to clients & attend mandatory trainings. Attends supervision with supervisor once a week. Escort clients to appointments, as needed. Support program clients in pre-crisis or crisis situations. Educate clients about recovery skills, based on shared life experiences. Help clients transition through recovery process. Share information with clients about benefits, entitlements, and other supports. Provides resources and referrals to help clients connect with desired services. Link clients with needed community resources and treatment services. Provide peer mentoring services; co-facilitate groups/community meetings and train clients in Narcan administration/FTS training. Collaborate with HOPE Housing case managers and other providers clients are connected with. Collaborate with external providers and case conference as needed Position requires a High School Diploma or equivalent. One year recovery time and CRPA/Certification as a Recovery Peer Advocate (Substance Abuse) required, or CRPA-p. Experience with population and substance use disorder (SUD) clients. Must be willing to disclose lived experiences in mental health, substance abuse and/or criminal justice. Must be able to navigate public transportation system. Bilingual Spanish/English a plus. Must have exceptional communication skills both orally and in writing, with individuals and groups regarding complex and sensitive issues. Physical ability to perform the essential job functions is needed. Flexible schedule to meet the needs of the program/clients. In addition, the Peer Advocate shall perform any duties as required to meet the needs of the program and the clients that we serve.
