out of 5 stars Saint James, NY Social Worker Full-time Social Worker for our skilled nursing facility. The ideal candidate will have experience in long-term care/ sub-acute setting and must have previous healthcare experience.
Social Worker Duties:
Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs Identifying individual social and emotional needs Developing and Maintaining individualized care plans Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotion needs of the resident
Social Worker Requirements:
