Queen City Hospice is hiring a Hospice Social Worker to supportMason and surrounding counties. Performs the psychosocial assessment of patients andfamilies/caregivers in the hospice program in a timely manner andensures that identified needs are addressed in the patient’s planof care. Provides an assessment in the patient’s identifiedresidence and assistance when this is not safe, and another plan isrequired Demonstrates exceptional service to patients, families,referral sources, co-workers, and other customers. Assist teammembers as needed to ensure that this level of service isconsistently provided Counsels patient and family/ caregivers andstaff as needed in relationship to stress, and other identifiedcoping difficulties Provides accurate and timely documentation ofpatient services to reflect the plan of care Provides assistancewith applications for Supplemental Security Income, Hospice,Medicaid, and Medicaid Waiver. Obtains resources from communitysocial, health, pharmaceutical and welfare agencies to assist inmeeting patient/family needs Serves as liaison between patients andfamilies/caregivers and community agencies
Qualifications:
SocialWork Degree Ohio:
Bachelor’s degree in Social Work; Master’s degreepreferred. The bachelor’s degree can be in Psychology, sociology,or other field related to Social Work. Current State Licensure in
Social Work Ohio:
