Social Work Full Job Description Responsibilities:
Performs 100% intake and psycho-social assessment on all patients and their families. Coordinates services and offers individual, family or group counseling and discharge planning. Acts as primary contact with third party payers and their representatives.
Requirements:
Must have a Masters degree in Social Work and be licensed to practice in New York. Relevant work experience in a hospital or nursing home setting .
