Social Worker Job Description Looking for a full-time Social Worker with the ability to evaluate and treat independently without direct supervision. Must possess the desire to assist residents to improve and should have a passion for working with geriatric population.

Social Worker duties include:

Complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs. Assists the Social Services Director in providing services to meet the social and emotional needs of the residents/patients. Develops a rapport with the residents/patients and presents their concerns to facility management. Charts as assigned. Ability to relate positively, effectively and appropriately with residents, families, visitors, and other facility staff. Ensure and provide support and education to residents/family members/significant others to assist in the understanding of placement and facility issues.

Social Worker Requirements/Qualifications:

Bachelor’s Degree required. Master’s Degree in Social Work or Human Services preferred. Experience in LTC through employment or internship required LMSW preferred

up to $64,350
