Champion Resident Well-being:

Conduct comprehensive psychosocial assessments, develop individualized care plans, and provide ongoing support to residents and their families.

Navigate Transitions:

Assist residents and families with admissions, discharges, and transfers, ensuring smooth and supportive transitions.

Resource Navigator:

Connect residents and families with essential community resources, entitlements, and support services to meet their evolving needs. Advocate with

Empathy:

Act as a strong advocate for residents’ rights and preferences, ensuring their voices are heard and respected.

Counsel & Support:

Provide individual and group counseling, crisis intervention, and emotional support to residents facing various life challenges. Collaborate for

Success:

Work seamlessly with an interdisciplinary team (nursing, therapy, activities, dietary) to ensure holistic, person-centered care.

Educate & Empower:

Educate residents, families, and staff on topics related to resident rights, advance directives, and psychosocial aspects of aging.

Maintain Excellence:

Document interactions and interventions accurately and timely, adhering to all regulatory guidelines and professional standards.

What You’ll Bring:

Seniors:

A deep-seated passion for working with the elderly population and a commitment to their dignity and quality of life.

Education & Licensure:

A Bachelor’s or Master’s degree in Social Work from an accredited program. Current licensure in Pennsylvania (LSW or LCSW) is highly preferred or eligibility to obtain within a specified timeframe. Experience that
