The Children’s Home of Pittsburgh is seeking a Social Worker
- Early Intervention Services (1099 Contract Worker) to join us in providing Early Intervention home based services to patients birth to age 3 in Allegheny County and/or Butler County. The Social Worker
- Early Intervention Services
- will provide comprehensive social services to children and families receiving early intervention services, birth to age 3.
The Social Worker will assist families in accessing resources, navigating systems and providing support and follow Social Work outcomes defined on the child’s current IFSP. The goal of a Social Worker is to help promote healthy child development by providing social work services within the home and family setting. Office space will be available as needed for administrative tasks related to caseload. Candidates must live in the Pittsburgh or surrounding areas.
Requirements:
Essential Functions:
Must have the flexibility in schedule to see clients when they are available, which may include later afternoon, evening and weekends. Social Worker EI service implementation of current Social Work outcomes on the IFSP, connecting with referral resources, documentation, follow up and discharge planning. Ensure required documentation is completed via electronic health record for all patients within specific timeframes. The social work services provided are consistent with best practice, evidence-based practice, and adheres to all policies, procedures, NASW Code of Ethics, and HIPAA. Ensure clear communication between the social worker and client/family or other members of the service team. Attends and participates in mandatory staff meetings and supervision. Demonstrate independence in professional growth through continuing education or other sources such as workshops, seminars, etc. Perform all other duties as assigned.
Minimum Skills and Abilities Required:
Master’s degree in social work from an accredited college or university. Active Pennsylvania Social Work License (LSW or LCSW). Interpersonal and communication skills necessary to effectively communicate and work with families and all staff including other therapists and members of the therapeutic team. Strong knowledge of community resources. Must have working knowledge of computers to complete documentation electronically and in a timely manner.
Functional Requirements:
Must be able to perform EI services a minimum of eight hours per day. Ability to communicate information in a professional and courteous manner with other staff and families. Manual dexterity necessary to operate computer and telephone systems, and other typical office equipment. Physical ability to perform job requirements of community social work. Ability to operate a motor vehicle and/or access other means of transportation in order to travel locally for business related to The Children’s Home. Displays strong decision making and problem-solving skills. Demonstrates professional behaviors.
