JOB SUMMARY:
Service coordination is an ongoing set of activities that assist infants/toddlers and their family/caregiver with gaining access to services and supports appropriate to their needs. Service coordination activities must be directly related to the management of services or supports for the eligible infant/toddler and their family/caregiver. Service coordinators are responsible for providing families/caregivers with unbiased information on child development, community activities, services, and resources on an ongoing basis.
DISTINGUISHING FEATURES OF THE CLASS
A person in this position should be well organized and able to take instruction well. They need to be able to work with clients to get them the best possible help and do what is best for them. The work is performed independently and under supervision when necessary. The Caseworker Supervisor is available when problems arise or a consult is needed.
JOB FUNCTIONS
Coordinating initial and ongoing eligibility evaluations and developmental assessments of children and their families; Screening and tracking children who are at-risk for developmental delays; Facilitating and participating in the development, implementation, and review of the IFSP. Assisting with identifying and gaining access to EI services and other supports identified on the IFSP. Assisting with identifying service providers and facilitating communication with chosen providers. Coordinating and monitoring the delivery of EI services; Informing families/caregivers of their rights and procedural safeguards, and advocacy services. Monitoring progress towards achieving the outcomes included in the IFSP. Assisting parents with arranging for health services their infant/toddler needs or is receiving. Supporting families/caregivers to participate in inclusive community activities and events Informing families/caregivers of community resources as needed; Providing information about local support groups and family/caregiver networks; Facilitating the development of the transition plan as part of the IFSP; Completing a service note after any contact with, or on behalf of a child and their family/caregiver. Performs other related work as required.
PHYSICAL AND MENTAL REQUIREMENTS
Must have the ability to record, convey, and present information, explain procedures, and follow instructions; Must be able to sit for long periods of time with frequent periods of standing, walking, and occasional twisting, reaching, pulling, and pushing required for obtaining files, doing inspections or other job duties; Dexterity requirements range from simple to complex movements of fingers/hands, legs/feet, or torso to carry out job duties; Sedentary work with occasional lifting, 15-20 pounds; Must demonstrate emotional stability; Ability to cope with physical and mental stress of the position; Must be able to pay close attention to detail and concentrate on work.

Have thorough knowledge of casework principles and methods. Have knowledge of social, economic and health problems and resources for assisting clients in these areas. Ability to plan and organize work and prepare adequate records and reports. Ability to understand and interpret laws and regulations in regard to BEISFS. Working knowledge of early childhood development MINIMUM EXPERIENCE and
TRAINING
Bachelor’s degree from an accredited college or university which includes 12 college credits in early intervention, early childhood special education, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology or other comparable human services and 1 year of full-time or full-time equivalent experience working with or providing case management to children, families or individuals. Or Associate’s degree or 60 credit hours, from an accredited college or university in early intervention, early intervention, early childhood special education, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology or other comparable human services and 3 years of full-time or full-time-equivalent experience working with or providing case management to children, families or individuals.
ADDITIONAL
EXPERIENCE and
TRAINING
Must possess current ChildLine, FBI and Pennsylvania State Police Criminal History Clearances. Must possess a valid driver’s license. Must be able to work flexible hours. Must possess Infant CPR and First Aid Certification. Must complete a minimum of 24 hours combined mandated and elective professional development annually. Must complete pre-service training completed prior to independent work with families.
