Addictions Counselor / Discharge Planner 3.9 3.9 out of 5 stars Collegeville, PA 19426 The Addiction Counselor will provide counseling and support to individuals struggling with addiction, helping them navigate the path to recovery. Prison Discharge Planner / Corrections Social Worker / Corrections Counselor / Drug Recovery and Resettlement Looking for a Social Worker, Human Services, Criminal Justice or Mental Health Professional with addiction experience to work directly with prisoners that have a history of drug abuse, assist with discharge planning, MAT therapy and appointments, coordinating community resources are in place for reentrants on the date of their release.

TRAINING

On-site training provided during shift times on location AND DOC training will be required with in the first 6 months of starting the role. This will be 3 weeks of required training to be completed in Elizabethtown, PA. Travel will be reimbursed Overnight stay will be provided to stay during the week, Saturday and Sunday will be at home.

Job Duties:

Implement and provide social work, counseling, and case management services to individuals to enhance their social functioning and to help them attain a more satisfactory social, economic, or physical adjustment. Assist with referrals in the community, including setting up and scheduling appointments for medication and treatment. Ensure needs, benefits, and services are obtained in accordance with assigned individuals’ treatment plans. Track and update spreadsheet/database on everyone released from the facility Meet with reentrants with positive urine drug screens to assess and refer to treatment. Complete assessments as required (Initial, 6 months, discharge). Communicate all treatment planning to Center Staff, SCI Staff, and/or Parole staff.

Requirements:

Bachelor’s in Social Worker or Human Service or Similar field required. Knowledge and experience in Case Management, Discharge Planning and Community Resources. Knowledge of counseling techniques and interventions used to recognize or improve psychosocial, economic, and physiological situations of individuals and families. Knowledge of commonly occurring physical and mental illnesses and their social, economic, and psychological implications.
