Jun 24, 2025 Description The Student Case Manager & Care Coordinator serves as the primary non-clinical point of contact for students navigating personal, academic, and social challenges that impact their college experience. Reporting to the Dean of Students and working in close collaboration with health services, counseling, academic advising, and other departments, this position ensures that students receive coordinated, compassionate, and comprehensive support. This pivotal role enhances student retention, success, belonging, and overall well-being by facilitating timely interventions, managing care coordination plans, and leading student support initiatives. The Case Manager is a key leader of the Student Support and Intervention Team (SSIT) and oversees the administration of support efforts for students of concern.
Key Responsibilities:
Serve as the primary case manager for students experiencing complex life circumstances, including current or emerging mental or physical health issues, financial hardship, family disruptions, and challenges with collegiate transition. Lead care coordination across campus departments and external providers to ensure a seamless support experience. Provide individualized assessments and formulate intervention plans tailored to diverse student needs. Maintain detailed records and ensure compliance with College policy, FERPA, HIPAA, and other legal guidelines as well as best practices with HECMA. Serve as the administrative leader of the Student Support and Intervention Team (SSIT) and a key member of the Behavioral Intervention Team (BIT). Coordinate and track student leaves of absence and return-to-campus plans, ensuring follow-through on all requirements. Facilitate communication with faculty, staff, and/or families, when appropriate, to support student care and recovery. Help students understand and utilize health insurance benefits and access local community resources. Maintain a directory of local service providers and strengthen partnerships with external agencies. Provide crisis intervention and follow-up for students returning from hospitalization or emergency situations. Oversee management of student emergency fund and ensure timely distribution and tracking. Design and implement processes to streamline referrals and follow-up support. Participate in campus life on-call rotation and respond to after-hours concerns on a limited basis (about 4x/semester). Support division-wide administrative processes and policy development. Represent the division at events, meetings, and professional conferences. Conduct program assessments and generate reports to evaluate the effectiveness of support services. Qualifications Required
- Bachelor’s degree in Social Work, Counseling, Higher Education, or a related field (Master’s preferred).
- Experience with case management in a college/university or young adult support setting.
- Demonstrated ability to maintain boundaries while fostering trust and support.
- Experience working with diverse populations and an inclusive approach to student care.
- Strong interpersonal and communication skills with the ability to manage sensitive and confidential situations.
- Proficiency in documentation, reporting, and using case management best practices.
- Ability to work both independently and collaboratively within interdisciplinary teams.
- Flexibility, problem-solving skills, and a non-reactive approach to service delivery.
- Comfort and experience with parent and family engagement in a collegiate context.
Preferred Qualifications:
- Experience with student leave of absence and return-to-campus protocols.
- Background in crisis response, risk assessment, or behavioral intervention support.
- Knowledge of student development theory, trauma-informed practices, or transition planning for young adults.
- Familiarity with local community resources and support systems.
