GPCA seeks a Family Empowerment Case Manager for our team.

Position Overview:

The Family Empowerment Service Case Manager is responsible for being an ambassador of GPCA’s mission, values, and commitment as they contribute to partnerships in an array of community settings. With the approach of working towards increasing organizational stability, breaking the cycle of intergenerational poverty, and having a greater impact on generational poverty. The Case Manager will assist children and families in ensuring household stability. Under the supervision of the FES Supervisor, the Case Manager will serve a pivotal role in ensuring that children, youth, and families are receiving appropriate and effective services to improve their safety and well-being. FES adheres to a strength-based management model that builds on each family’s unique strengths to help them resolve problems. The case manager will collaborate with families to identify the barriers and link them to community-based resources that will help remove/alleviate the barriers.

Key Responsibilities:

Represent GPCA’s mission, values, and commitment in all Community settings Adhere to the agencies Policies & Employee Handbook, FES Program Standards, and professional code of ethics Carry a caseload of up to 15 families through home visiting with a maximum capacity of 20 as needed. Engage all families within 5 business days of receiving a FES referral Maintain ongoing communication with partners at the Department of Human Services Conduct an intake and create a family service plan that will address identified barriers Engage with families in a collaborative relationship that will enhance the potential for the client’s completion of service with positive outcomes through bi-weekly home visits Maintain an awareness of community resources, facilitate, and monitor student/family access to appropriate resources and services necessary to support and supplement service goals. Conduct 2 monthly home visits in which the children and caregivers are present Assess and ensure the safety and well-being of each child in their home environment a minimum of once per month Conduct a school visit to collect attendance record for school age children in the home within 15 days of intake. Ensure all children in the home are up to date with their medical & dental care and school attendance when needed within 10 days of intake Document all contacts with or on behalf of assigned families in the database management system within 6 days of contact Maintain case files with all program Intake paperwork as required by FES Program Standards Adhere to GPCA and regulatory entity guidelines concerning caseload requirements, service delivery, accountability, and confidentiality. Adhere to the 20 hour per year training requirement established by Philadelphia DHS and GPCA standards Attend bi-weekly supervision and all department meetings Allow flexibility in daily schedule to accommodate events on an as-needed basis. Adhere to the GPCA attendance policy and communicate PTO usage/request to the FES Supervisor Document daily activities on Outlook calendar and share the calendar with the Supervisor Conference any child safety concerns with the FES Supervisor for assessment and make a report to Childline as needed Complete all other tasks as assigned

Requirements:

Bachelor’s degree in social work or human Services field (required) 2+ years of social service experience or 1+ year of experience plus 1+ year of internship Working understanding of child development and family dynamics Working understanding of community and resource development Ability to work collaboratively in a multi-disciplined environment Excellent written and verbal communication skills Creative, outgoing collaborator with the ability to adapt quickly to changing needs and priorities Strong time management skills Ability to work with a diverse population Must be detailed oriented and highly organized Proficiency in MS Office Applications (Word, Outlook) Working knowledge of database applications Bilingual- Spanish/English (preferred, but not required)
