Social Worker Delaware Valley Skilled Nursing & Rehabilitation Center 111 Rivers Edge Dr, Matamoras, PA 18336 Job Overview We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing support and assistance to individuals and families facing various challenges. This role requires a strong understanding of social work principles, crisis intervention techniques, and the ability to work collaboratively with other professionals in a healthcare setting. Duties Providing counseling and emotional support to residents and their families Advocating for residents’ rights and addressing concerns Assisting with administrative tasks, including documentation and quality improvement Coordinates all aspects of patient and resident admissions to the center, working cohesively with care team members for a safe, seamless transition liaison between all departments, residents, and families Maintains effective communication with residents, families, healthcare professionals and facility staff Experience working in a long-term care or similar healthcare setting preferred Strong communication, interpersonal, and organizational skills Ability to work effectively in a team-oriented environment If you are passionate about making a difference in the lives of our residents and their families, we encourage you to apply for this rewarding opportunity. Experience An individual with a minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, rehabilitation counseling, and psychology. One year of supervised social work experience in a health care setting working directly with individuals.
