Our mission focuses on reducing recidivism, stabilizing families, providing housing opportunities, and promoting community investment through holistic reentry programs. Summary As a Case Manager at OpenDoors, you will play a vital role in supporting formerly incarcerated individuals as they transition back into society and helping individuals struggling with homelessness find housing. Your work will involve providing comprehensive case management services that facilitate successful reintegration and promote personal responsibility. This position will be split 50/50 between our Resource Center and Women and Children’s Recovery House. There is the possibility that the location of this work could change in the future as OpenDoors has multiple different locations that require case management work. Responsibilities Conduct assessments to identify the needs and strengths of clients transitioning from incarceration or individuals struggling with homelessness. Develop individualized service plans that outline goals and necessary resources for clients. Address client needs in the areas of housing, addiction recovery, parenting, employment, transportation, and health. Coordinate with community resources, social services, and other stakeholders to facilitate access to essential services. Advocate for clients by addressing barriers to re-entry and promoting their rights within the community. Meet regularly in a one-on-one session with each active client Provides referrals to appropriate other service providers within the agency and externally. Maintain professional rapport with service users, peers, and other community service providers. Uses assessment information to establish client centered goals, develop service plans which contain specific measurable objectives and timeframes for completion. Communicates with internal and external departments regarding case coordination and complete safety plans and risk minimization to appropriately support clients/residents including probation departments, housing providers, and DCYF. Provides referrals to appropriate other service providers within the agency and externally. Attend all required training and participate in voluntary training as needed.
Including but not limited to:
Certified Peer Recovery Coaching, Motivational Interviewing, De-Escalation, Trauma Informed Care, NARCAN, Mental Health First Aid. Maintain all administrative duties, including but not limited to: maintaining current documentation for program eligibility and compliance, electronically and on paper as well as documentation related to time and effort. Assist clients with transportation–case managers may have to give rides to clients in their car at times. Assist clients to progress towards permanent supportive housing, including helping secure housing opportunities and vouchers. Facilitate the client towards becoming independent financially and in their recovery, and assist them in developing a personally directed, community directed recovery with multiple supports. Track client progress with case notes and records and reports as needed. Track client progress in the Homeless Management Information System (HMIS). Complete all necessary enrollment, planning, and data tracking responsibilities necessary for enrolling and billing for clients into Housing Stabilization Services. Upon potential Certification as a Peer Recovery Coach, complete all necessary peer recovery coaching enrollment, planning, and data tracking responsibilities necessary for enrolling and billing for clients into Certified Peer Recovery Coaching Services. Requirements Bachelor’s degree in social work, psychology, or a related field preferred. Experience in case management or working with at-risk populations is highly desirable. CPRS certified or ability to obtain certification. Strong understanding of behavioral health and addiction counseling principles. Excellent communication skills, both verbal and written. Ability to work collaboratively with diverse populations and community organizations.
Note:
