Non-Exempt Summary of the
Position:
Intensive Family Coordinator provides a professional level of case management services as well as direct advocacy services to a caseload of families. Intensive Family Coordinator will be responsible for completing family assessments, recruiting foster parents, completing home studies for the referring authority, locating family resources, and creating individualized service plans. Case management services including intakes, safety planning, team meetings, ISP development 24/7 support, case contacts, and case record documentation in keeping with state regulations.
Qualifications/Requirements:
High School Diploma or GED is required. Experience with community work and at least one (1) year experience working in human services field. Experience working with at-risk youth and families Experience in the provision of home and community based services. Strong verbal and written communication skills Position requires travel; reliable transportation, valid driver’s license, current auto insurance Bilingual/Spanish speaking is a plus
