At the time of an offer, determination of your offer will reflect your skills and experience as it relates to the position.•The Social Worker provides social services to resident/families at the time of admission, during their stay at the nursing home, prior to discharge, and provides timely follow-up services in the community.

Essential Duties and Responsibilities:

Assists in the application process by sending and receiving applications. Will contact prospective resident or responsible party. Contacts social services representatives and families. Assists residents to exercise their rights as citizens. Develop a social history on each new resident. Document when necessary, in resident’s medical records. Participate in regular quality assurance interdisciplinary team meetings. Identify problems and develop and implement studies relative to issues affecting residents. Propose recommendations resulting from quality assurance data. Work with the interdisciplinary team and administration to promote and protect residents’ rights and the psychosocial well-being of each resident. Provide time-limited counseling services to residents as deemed appropriate. Discharge planning and referrals to community resources, as appropriate. Will assist in admissions during an absence of the Director of Admissions. Assembles admission packets. Assists with all pre-admission paperwork and forwards to appropriate party. Assures that all information is complete on admission record. Assists in orienting resident to facility. Will carry out other duties, as they relate to social services, as needed.

Demonstrates knowledge about the Material Safety Data Sheets book. Adheres to the policies and procedures set forth in the Personnel Handbook. Adheres to Campus Organizational Integrity Model and practices Mount St. Rita core values of respect, integrity, compassion, and excellence. Punctually starts workday and does not abuse PTO days, break time, or overtime. Dresses in a professional manner. Attends mandatory in-services and other in-services in order to update and enhance present knowledge and skills. Seeks out opportunities for professional growth.

Qualifications:
Education:

Bachelor’s degree in social work or related field (i.e. psychology). Ability to deal with a variety of situations involving resident/family, staff, and Community agencies. Good oral and written communication skills.

Licensure:

Must have an active social work license in Rhode Island.

Training:

Should have a minimum of two years experience as a social worker in a Skilled nursing facility or other related facility.

Job Knowledge:

Must be able to identify the medically related needs of the residents and the services provided to meet them when the resident is admitted. Understands the physical and emotional problems and social needs of the aging residents and assists resident/family in dealing with them.

Physical Demands:

Must have good general health and demonstrate emotional stability, have flexibility, function, independently, have personal integrity, and the ability to work well with others. Must be able to work with the ill, disabled, elderly, and emotionally upset. Availability for emergency counseling as needed.

Special Demands:

Must have the ability to deal with a variety of situations, in many contexts. Must have the ability to prepare reports and to evaluate and present workable service plans. Must realize the importance of strict confidentiality concerning private information of residents, their families, and employees of the nursing home.

Working environment:

Works in well-lighted, ventilated areas. Sits, stands, bends, lifts, and moves intermittently. Subject to frequent interruptions. Involved with residents, families, personnel, visitors, etc. Communicates with other departments. May be exposed to residents who have communicable diseases.
