Licensed social worker needed for MUSC Children’s Health Wellness Works program, a comprehensive pediatric healthy lifestyle clinic. We see patients 2-22 years old and help children and their families develop healthy habits to manage weight and comorbidities such as diabetes, high cholesterol, and high blood pressure. A social worker is needed to complete needs assessments for our patients and their families as well as provide counseling and psychotherapy for patients who have diagnosed mental health conditions.

Job Description

This role is eligible for a hybrid work schedule.

Job Responsibility:

Collaborates with other professions to develop, implement & follow-up on the Psychosocial Plan of Care.
The Wellness Works Licensed Independent Social Worker focuses on pediatric emotional and behavioral needs, in addition to familial support through psychoeducation and family therapy.
Conducts a psychosocial assessment to understand family dynamics and social challenges. Screens patient for eating disorders.
Engages and empowers patient/family in problem solving and decision making regarding their health care decisions as evidenced by documentation in the medical record and/or team observation/feedback.
Demonstrates problem solving and critical thinking through reassessment of the patient’s plan of care, adjusting goals and interventions as needed.
Provides assessment and intervention with cases involving suspected child abuse/neglect, medical abuse, sexual assault, adult/elder abuse, and domestic violence. Contacts Adult/Children’s Protective Services, as appropriate and in accordance with the MUSC process and state law, as evidenced by documentation in the medical record and/or team observation/feedback.
Based on assessment, communicates recommendations with the patient, family, and interdisciplinary team to support a collaborative, patient-centered approach. Ensures effective communication through patient hand-offs with interdisciplinary team as evidenced by documentation in the medical record.
Develops an action plan with the patient, family, and team taking into consideration variables that impact the patient’s care to include social, emotional, physical, or environmental factors as evidenced by documentation in the medical record.
Responds to consults based on positive screenings for both depression and suicidal ideation, completes assessment as needed and takes appropriate steps based on outcome. Steps may include a referral to psychiatry services if needs exceed social worker’s capacity to treat.
Assesses patients for barriers to care otherwise known as the Social Determinants of Health (SDoH) on an annual basis, minimally.
Makes regular telephonic contact with patients who are moderate to high risk for readmission.
Demonstrates knowledge of community resources by effectively evaluating, based on psychosocial assessment, and selecting appropriate resources based on patient/family need. Additional Job Description
