This position is approximately 80% clinical direct patient care and 20% administrative. Under clinical practice guidance of the Social Work Manager, the Psychiatric Social Worker Lead assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes.
