This individual would be responsible for identifying and providing for each resident’s social, emotional and psychological needs. Other responsibilities include assisting with planned discharges, as our residents transition back to the community.

Full Time Qualifications Some knowledge of PCC Health care experience in long-term care Some knowledge of State and Federal guidelines Must be able to relate to residents and families Have the ability to work cooperatively with other employees, personnel, and community agencies.
