Monte Nido’streatment approach focuses on the restoration of physiological andnutritional balance, implementation of healthy eating and exerciseroutines, elimination of harmful behaviors, and development ofmotivation and treatment engagement. Our objective is to help eachclient achieve a clear understanding of their eating and/orexercise disorder and its effect on their life, as well as anindividualized appreciation of what is necessary for their personalrecovery. We are seeking a Hybrid Social Worker to join our team.
The Social Worker will play a key role in providing carecoordination and case management across all Monte Nido Oregon sites, serving both adults and adolescents and their families in ourIntensive Outpatient (IOP), Partial Hospitalization Program (PHP),and Residential Treatment Center (RTC) programs. This role requiresa dedicated professional to support the clinical team in ensuringsmooth transitions between care levels and assisting clients andtheir families in navigating and accessing essential communityresources.
Schedule:
Monday-Friday 8:30 am-5 pm (Hybrid position)

Responsibilities Include:

Care Coordination & CaseManagement:
Partner with the clinical team to offer case managementand care coordination for clients and families, ensuring seamlessaccess to community resources and services.
Community & FamilySupport:
Assist in connecting clients and families with relevantcommunity-based programs, educational resources, and supports frominsurance or government programs, including but not limited to flexfunds, transportation to care, and SNAP benefits.
ReferralManagement:
Send referrals to community programs and resources, aswell as help facilitate collaboration with outpatient providers andwraparound teams.
Transitions Between Care Levels:
Help facilitatesmooth transitions between various levels of care, such as fromPRTS to community-based services, ensuring continuity of care.
Prepare Clients & Families for
Transition:
Provide necessaryinformation and emotional support to both the client and family toprepare for these transitions.
Ongoing Resource Connection:
Ensurethat clients continue to receive the care they need aftertransitioning by connecting them with appropriate resources andservices in their new care setting, following up post dischargefrom a Monte Nido Program. Perform other duties as assigned.
Qualifications:
: Master’s degree in Social Work or related fieldLCSW preferred Experience with CareOregon/Medicaid is a must Provenexperience in case management or care coordination, preferably inbehavioral health or residential treatment settings. Strongcommunication skills and the ability to work collaboratively with amultidisciplinary team. Ability to build relationships withclients, families, and community providers to enhance client careand ensure successful transitions.
