Social Worker Caring House Calls LLC Florence, SC 29505 • Hybrid work Job Summary We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will possess a strong background in mental health counseling and case management, providing support to individuals facing various challenges. This role requires a commitment to improving the well-being of clients through effective assessment, counseling, and advocacy. Duties Conduct comprehensive patient assessments to identify needs and develop individualized care plans. Provide psychotherapy and mental health counseling to clients in both individual and group settings. Maintain accurate medical records and documentation of patient interactions and treatment plans. Implement cognitive behavioral therapy techniques to assist clients in overcoming challenges. Collaborate with multidisciplinary teams to coordinate patient care and ensure holistic support. Advocate for clients with disabilities, ensuring access to necessary resources and services. Monitor client progress and adjust treatment plans as needed based on ongoing assessments. Educate clients and their families about mental health issues, treatment options, and coping strategies. Requirements Master’s degree in Social Work (MSW) from an accredited institution. Experience in case management and working with individuals with disabilities is preferred. Proficiency in medical terminology and understanding of healthcare systems. Strong skills in psychotherapy, mental health counseling, individual group counseling, and patient care. Ability to conduct thorough patient assessments and develop effective treatment strategies. Excellent communication skills, both verbal and written, with the ability to build rapport with clients. Commitment to maintaining confidentiality and adhering to ethical standards in social work practice.
