This position coordinatesadmissions/discharges of residents in healthcare and maintainsappropriate contact with hospitals, doctors, and outside agenciesto facilitate admissions. We look forward to meeting you soon!

POSITION SUMMARY
: Coordinates orperforms the necessary administrative functions regarding theadmissions/discharges of residents in healthcare. Assist in theorganization and planning of the Social Services provided forresidents of this community.
ESSENTIAL FUNCTIONS
: ? Assists withthe responsibilities and functions delineated in the dischargeplan. ? Ensures that the administrative and clerical functionsrelated to the admission and/or discharge of residents is carriedout to completion by coordinating efforts with the Healthcare TeamMembers. ? Maintains appropriate contact with hospitals, doctorsand outside agencies to facilitate admissions. ? Coordinates allforms needed for admissions. ? Maintains state required residentroster. ? Coordinates financial updates for private pay residents.? Completes appropriate section of the MDS. ? Participate andattend all in-service training and education programs as scheduled.? Other duties as assigned.

Full Physical Demands, Work Environment, andExpected Hours of Work:
The physical demands described here arerepresentative of those that must be met by an employee tosuccessfully perform the essential functions of this job. ? Thisposition is very active and requires frequent standing, walking,bending, kneeling, stooping and climbing. ? This position will berequired to lift or carry weight up to 50 lbs. and ability to pushup to 250 pounds independently. ? The individual must use properbody mechanics to assist residents in their daily living. ? Thisposition regularly requires long hours and frequent night andweekend work.
T RAVEL:
No travel is expected although occasionaltravel may be required for training sessions, continuing educationopportunities, emergency situations, and other company functions.
EDUCATION AND EXPERIENCE REQUIREMENTS
? Bachelor’s degree inSocial Work, or closely related human services field desired. ? 1-2years related experience in a Healthcare setting. ? Two years’experience in a similar social work position desired. ? Two yearsof long-term care or rehabilitation experience desired.
K NOWLEDGE, SKILLS, AND ABILITIES REQUIRED
? Ability to read, write,speak and understand English fluently. ? Ability to meet or exceedthe company’s attendance and punctuality standards. ? Ability touse Electronic Records and miscellaneous software and officeequipment. ? Ability to understand and follow directions as given.? Ability to work with minimal supervision.
