- The social worker is responsible for developing the trement plan and setting achievable trement goals with the Veteran and family. The trement plan completed and upded as specified by policy for the assigned work area. Referral to service providers
- Throughout the course of trement the social worker is the subject mter expert on VA and/or community resources. The social worker will collabore with other service providers in reassessing the Veteran’s needs. Resource Usage/Development

- The social worker will review the progress notes from the other providers to accurely determine the strengths and limitions of each Veteran being referred for mental health services. - Social worker will provide the Veterans and their caregivers with ongoing supportive counseling.
Administrive Responsibilities:
Enter all Veteran/family contacts in the electronic record using approprie forms and temples. This informion entered in a complete, confidential, and professional manner to ensure informion on the pient is shared with other VA staff. This informion reviewed on a regular basis. Establishes and maintains positive working relionships with employees, volunteers, consumers and stakeholders within the VA and outside community agencies. Attends all approprie staff meetings (interdisciplinary trement team meetings, Mental Health Social Work staff meetings, committee meetings as approprie, etc.) and perform other duties as required. Collabores in the performance improvement processes and comply with performance measures as required by the VA for their specific populion and/or program. Coordines the program operion of the inpient acute psychiry unit, and programmic activities of professional and multidisciplinary staff with various tours of duty. Assists in planning, establishing, and implementing policies and procedure, developing individual or group goals and objectives as well as monitoring, opering, evaluing, coordining, and overseeing the clinical activities of the psychiry units. Ensures the clinical staff are aware of and adhere to all current directives of VHA, and meet current JCAHO standards, Ste licensure standards, and VA-OIG expections.

Pay:
Competitive salary and regular salary increases. When setting pay, a higher step re of the approprie grade may be determined after considerion of higher or unique qualificions or special needs of the VA (Above Minimum Re of the Grade).
Paid Time Off:
37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
