Tracking all suicide tempts and pients with serious suicide ideion through the “Suicide Behavior Report”. Provides comprehensive case management by ensuring all Veterans identified as high risk for suicide receive follow up mental health care as required, coordining services as needed. Provides crisis intervention, suicide risk assessments, and safety plans for Veterans via phone and in person. Coordining with mental health staff and primary care staff through ongoing communicion to ensure th necessary components in the veteran’s care plan are addressed to include a suicide risk assessment and safety plan. Completes behavioral health autopsies as required by the Nional Suicide Prevention Program. Following completed suicides, provides postvention services to both surviving family members and VA staff involved in the affected Veteran’s care. Participion and provision of expertise on individual root cause analysis teams. Assuring th all Veterans on the facility tracking list are being followed closely by the approprie provider on a regular basis as determined by the coordinor and the provider. Tracking any no-show appointments by these Veterans and following up with him or her as soon as possible to reschedule the appointment. Initiing emergency interventions as approprie to the situion, including but not limited to, contacting the Veteran’s identified emergency contact and/or 911 if the safety of a Veteran is unable to be established, or it is determined th the Veteran is imminent risk for self-harm. Dissemining, organizing, and where possible, conducting evidence-based and promising/emerging interventions. Maintaining awareness of suicide trement options and making recommendions to the Veteran’s provider when new options become available. Assuring th Veterans have a 24-hour resource number to call and ways to get assistance if an immedie crisis should occur. Assuring compliance with Joint Commission standards reled to suicide prevention. Establishing an on-going Gekeeper Training program with non-clinical frontline staff such as clerks and telephone operors to assure th they are aware of options for Veterans they encounter during crisis situions. Working with community resources such as Veteran Service Organizions and other community groups to assure th they are aware of how to recognize Veterans risk for suicide and their options for getting these Veterans assistance. Responds to Suicide Prevention Hotline consults, interfacility consults, and phone inquiries from VA staff/community stakeholders to address needs of•risk Veterans. Ensures pient and staff educional merials on suicide prevention are prominently displayed throughout the facility and CBOCs. Ghers da regarding suicide tempts, completions, and staff/community training for recurring suspense reporting, adhering to stion and VISN reporting requirements, and timelines. Performs other reled duties as assigned.

Pay:

For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent least 25% of the time. Senior social workers are licensed or certified to independently practice social work an advanced level. Senior social workers typically practice in a major program area such as but not limited to: Polytrauma Rehabilition Center or Polytrauma Network Site; a Spinal Cord Injury Rehabilition Center, or a nional VHA referral center, such as a nional Center for Post-Traumic Stress Disorder or a nional Transplant Center, or other program areas of equivalent scope and complexity. The senior social worker may be assigned administrive responsibility for clinical program development and is accountable for clinical program effectiveness and modificion of service pterns. Assignments include clinical settings where they have limited access to onsite supervision such as CBOCs or sellite outpient clinics. The senior social worker collabores with the other members of the trement team in the provision of comprehensive health care services to Veterans, ensures equity of access, service, and benefits to this populion, ensures the care provided is of the highest quality. The senior social worker provides leadership, direction, oriention, coaching, in-service training, staff development, and continuing educion programs for assigned social work staff.
