JOB SUMMARY
The social worker evaluates a patient’s social, emotional, environmental, financial, and support needs. Assesses, plans and acts in response to the psychosocial needs of patients/families. Intervenes in crisis situations to assist patients. Facilitates decision making on behalf of the patients and families in complex cases. Collaborate with care manager in assessment and coordination of discharge planning. Provides interventions designed to promote health, prevent disease, and address barriers to access to healthcare.
JOB REQUIREMENTS
- Licensed Social Worker in TN Required
- Bachelor’s in Social Work Required
- 1 Year of Social Services Experience in Hospital Preferred
