Job Description:
As a member of the Clinical Care Team , the Social Worker will support primary care referrals and collaborate closely with providers to ensure continuity of care.
The role includes:
Key Responsibilities:
Case management: Connect patients to services (housing, food, mental health, etc.) Assist in obtaining medical equipment and supporting care plan follow-up Maintain and update a directory of community resources Provide patient education and relevant referrals Conduct in-person or virtual social needs assessments Document all patient interactions and coordinate with the clinical team Support patient outreach efforts, including high-risk and lost-to-follow-up patients Travel within the region as needed
Additional Expectations:
Participate in staff and community meetings Complete onboarding/orientation as directed by Regional Leadership Help remove barriers to care and build community partnerships
Qualifications:
Education & Experience:
Bachelor’s, Master’s, or Doctorate in Social Work from an accredited institution One year of full-time professional social work or related experience
Substitutions:
Graduate coursework (e.g., 36 quarter hours) in social work may replace the one year of experience Public health experience involving client services, outreach, or case management may also qualify OR Possession of a CMSW or LCSW license from the
