Full Job Description Overview:
Join our team at TrustPoint Rehabilitation Hospital as an Inpatient Rehabilitation Social Worker! Specializing in neuro and ortho, our patients are referred by outside hospitals in surrounding areas to our in-patient setting. We offer occupational, physical, speech, and respiratory therapy, in addition to other services like IV therapy, wound care, and rehabilitation nursing.
Our goal is to improve the quality of life by treating patients who are recovering from injury, illness, traumatic brain injury, spinal cord injury, amputation, stroke or disabled patients through the therapeutic use of everyday activities. Murfreesboro Rehabilitation Hospital is located within TrustPoint Hospital offering the only acute, hospital-based inpatient physical rehabilitation in Rutherford County.
PURPOSE STATEMENT
Plan, direct, define and coordinate individualized and group therapeutic support and services to patient’s with mental health, behavioral health and substance use issues.
Responsibilities:
ESSENTIAL FUNCTIONS
Conduct social history assessment of the patient through observation and patient/family interview, obtaining information on the patient’s needs, skills, interests and limitations for the development and implementation of the treatment plan. Coordinate and plan programs and activities to meet patient’s social and emotional needs. Promote recovery, symptom reduction and the achievement of the highest level of social functioning. Provides crisis management and family sessions to assist the family in understanding the implications and complexities of a life situation and its impact on lifestyle. Assist in the development of effective treatment plans that apply therapeutic principles appropriately while ensuring safety at all times for the patient. Participate in multidisciplinary treatment team meeting to discuss patient assessment, treatment plan and ongoing discharge plan. Communicate or aid family members in understanding and supporting the patient. Manage a caseload and assume responsibility for patient chart documentation. Facilitate education, process groups, support groups and referrals as requested. Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.

Qualifications:
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS
Bachelor’s degree in Social Work from an accredited college or university, including new graduate required. One or more years’ experience working with patient population preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS
Eligible for state licensure as one of the following: Licensed Social Worker (LSW), Licensed Associate Counselor (LAC), Certified Addictions Counselor (CAC), Registered Addiction Counselor (RAC), as required by the state in which the facility is operating. CPR and de-escalation/restraint certification required (training available upon hire and offered by facility). First aid may be required based on state or facility.
