Interviews and assesses patients and/or patient’s family, caregivers, and/or legal representatives.

Drag Edit DeleteParticipates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient’s treatment plan.

Drag Edit DeleteAssists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.

Drag Edit DeleteTakes Lead role in social work support for psychiatric patient population.

Drag Edit DeleteSupervises and/or trains new staff, students, and interns.

Drag Edit DeletePerforms Medicaid screenings as indicated for NHP.Drag Edit DeleteAssists patients with Safety Net applications when needed.

• Minimum Education
• Master’s degree in Social Work

• Required Skills
• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
