About Us Princeton Transitional Care & Assisted Living is a partnership between Signature HealthCARE and Ballad Health bringing a new level of health care services to the Tri-Cities area. The new Princeton Transitional Care will provide a new home for skilled-nursing care and will add 60 assisted living apartments, 47 short-term skilled-nursing and rehab private suites. Additionally, we have been awarded as a certified Great Place to Work for three years in a row and Modern Healthcare’s “Best Places to Work!” Overview As a Social Service Staff you will assist the Social Services Director/Coordinator in identifying and providing for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge. How you Will make a difference Assist residents and their families with social, emotional, and family issues. Assist in prividing timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies. Participate in resident care planning and discharge planning. Serve as a general source of information for residents and their families What you Need to make a Difference At least one (1) year of experience working in social work, preferably in healthcare setting. High level of professionalism and ability to maintain confidentiality. Effective Verbal and Written communication skills.
