Child Care and Youth Programs; +1 Extension and Outreach Job Description The purpose of this classification is to provide educational support and counseling activities to families with risk factors enrolled in the evidence-based Parents as Teachers home visitation program. Focus to be on healthy pregnancies, parent-child interaction and development-centered parenting and family well-being. This is a grant-funded position and employment is contingent upon grant continuation. Responsibilities Provide educational support and counseling activities to parents with fidelity to Parents as Teachers (PAT) evidence-based program model.
- Deliver all components of the Parents as Teachers model using the PAT Foundational Curriculum, implement PAT Essential Requirements and Quality Assurance Guidelines with fidelity to plan and deliver home visiting services to qualifying families.
- Maintain required caseload of families, conducting personal visits with each enrolled family one to two times per month.
- Conduct family assessments and screenings which include evaluating factors related to prenatal health, parenting behavior, and early development progress. Provide personal home visits that facilitate, reflect and partner with families, encouraging empowerment and self-efficacy; serving as mentor and coach.
- Refer individuals to community agencies based on needs and problems identified in assessments; coordinate access to needed private and public services and resources; follow up on referrals made and actions to be taken by clients.
- Plan, conduct and participate in educational parent group activities and/or targeted community-based events, including some after-hours and/or weekend events.
- Actively recruit families to participate in PAT program; coordinate planning, implementation, and evaluation of outreach events to build PAT presence and reputation in the community.
- Complete required national PAT training and annual competency-based professional development hours to maintain national PAT Parent Educator Certificate.
- Collect data requested by PAT National Center and program funders; complete all documentation and meet required documentation timelines regarding home visits, attendance records for PAT events, screening summaries, and other reports as requested by Supervisors.
Qualifications Bachelor’s degree in a relevant field and two years of relevant experience, or an equivalent combination of education, training, and experience. Three years of home visitation service experience preferred.
Completion of Evidence-based home visitation training (esp. PAT)
Knowledge of child growth & development and parent-child relationships.
Working knowledge of local social services, family and educational resources.
Proficient use of technology used for communication and data collection.
Ability to work cooperatively as part of a team and work independently.
Ability to think critically to solve problems.
Ability to maintain confidentiality.
Ability to perform moderate lifting of supplies.
Ability to get into and out of floor.
Prolonged sitting, especially in a driving capacity
Valid Driver’s license and responsible mode of transport required.
Must obtain Infant Mental Health endorsement within first year of employment.
