Overview:

We’re looking for a Per Diem Social Worker to join our hospiceteam. -The Social Worker (BSW or MSW) is that member of theinterdisciplinary team whose primary function is to providepsychosocial support to the patient/family unit based on anassessment that identifies the needs, goals, interventions andservices indicated accordance with the established plan of care andto utilize professional training and personal judgment inmonitoring the psychosocial assessment process.

About You:

Bachelor’s or Master’s Degree in Social Work from a CSWESchool Licensue as required by the state in which the hospice islocated One year experience as a Social Worker in a healthcare and/orhospice setting is required; three to five years preferred Knowledge of terminally ill patients and their families alongwith Understanding of hospice and the psychosocial dynamics ofillness, loss, and death; ability to apply knowledge of the specialneeds of hospice patient and families; sensitivity to the impact oflife and death issues face by individuals with terminal illness;tact in dealing with these patients and their families; ability tosupport Others at a time of crisis; ability to cope with the stressof repeated loss
