Posted 06/10/2025 Gifford is seeking a compassionate and skilled Social Worker to join our inpatient team. You will work with patients facing complex challenges such as chronic illness, homelessness, psychiatric conditions, substance use, and family or safety concerns. Your role will include identifying high-risk factors, coordinating family/team meetings, and supporting care planning through clinical evaluation and consultation. Collaboration is key, as you’ll work closely with other healthcare providers and community partners. You’ll also participate in hospital and community-based committees, helping to shape effective, patient-centered care.
Requirements:
