YOUR ROLEAs aSocial Worker with Evergreen Nephrology, you influence and ensurethe best clinical care for your patients. You leverage anintegrated technology platform and are complemented by an entireinterdisciplinary team including registered nurses, nurse caremanagers, health coaches, dieticians, pharmacists, psychiatrists,advanced practice clinicians, other social workers, and even othermedical specialists – all within the Evergreen Nephrologyfamily.
PRIMARY FUNCTIONSAs a Social Worker with EvergreenNephrology, you will own the behavioral health care plan for yourpatients, partnering with the Behavioral Health Advance PracticeProvider or psychiatrist and the interdisciplinary team to ensurethis aspect of the care plan is integrated into the full careplan.
Comprehensive management of behavioral health needs, includingassessment, diagnosis, treatment, and consultation, on alongitudinal basis, which minimizes the avoidable hospitaladmissionsProvide transitions of care support via collaborativepathways with inpatient, post-acute care facilities, and dialysiscenters, to ensure strong social support and behavioral health careplans as part of the overall transition of the care planPartnerwith Evergreen Nephrology’s care team to identify and eliminatesocial determinants of healthcare for your patients, includingfunctional capacity, home environment, health literacy, housinginstability, food insecurity, and caregiver support and in-homesupport service in care planningParticipate in ongoing scheduledconsultation with an interdisciplinary team to monitor patientprogress.

Link patients to community resources asappropriateSupport patients through direct, telephonic and videobehavioral health care for patients with a higher level ofbehavioral health acuityAccurately identify and document behavioralhealth diagnoses and conditions and establish behavioral healthtreatment plans for the respective conditions.
Implement evidencedbased psychotherapies.
Cogently explain the Evergreen Nephrology IDTresources and clinical programs to patients and families; guidepatients toward quality-of-life decisions aligned with patientgoals and values. Recognize goal-directed care in response topatient needs is an important component of managing psychiatricconditions.
