As a hospice medical social worker, you will beresponsible for psychosocial evaluations, and ongoing counse ofpatients and families during their end-of-life journey. Working inaccordance with the plan of care, you will provide emotionalsupport to patients and families when it’s needed most. Qualifications:
A heart to serve patients and families and a passion forproviding the best possible care
Education:
MS degree in social work from an accredited schoolof social work approved by the
Council of Social WorkEducation Licensure:
Current state license as a social worker
Experience:
2+ years of clinical work experience, preferably inhealthcare or hospice
Required:
Reliable transportation. Ability to sit, stand, bend,move intermittently and lift at least 25lbs and bear the weight ofan average adult effectively.
