Contract Medical Social Worker (MSW) IEase Home Care LLC 544 Straits Turnpike, Watertown, CT 06795 Overview We are seeking a dedicated and compassionate Medical Social Worker to join our healthcare team. The ideal candidate will play a crucial role in providing support and resources to patients and their families, helping them navigate the complexities of medical care. This position requires a strong background in social work, clinical counseling, and case management, with a focus on behavioral health and medical documentation. Responsibilities Conduct comprehensive assessments to determine patients’ needs and develop individualized care plans. Provide clinical counseling to patients and families, addressing emotional, social, and financial challenges related to their medical conditions. Collaborate with healthcare providers to ensure coordinated care and effective communication across systems. Maintain accurate medical documentation in compliance with regulatory standards and coding requirements. Engage in case management activities, including resource identification, referrals, and follow-up services. Participate in interdisciplinary team meetings to discuss patient progress and adjust treatment plans as necessary. Conduct diagnostic evaluations for children and adolescents, providing insights into their behavioral health needs. Stay informed about community resources and support services available for patients. Qualifications Master’s degree in Social Work (MSW) from an accredited program. Valid state licensure as a Clinical Social Worker (LCSW) or equivalent certification. Experience in social work within medical or clinical settings is preferred. Strong knowledge of behavioral health practices, case management principles, and medical documentation requirements. Excellent communication skills with the ability to empathize with patients from diverse backgrounds. Proficiency in research methodologies related to social work practices is a plus. Experience working with children and conducting diagnostic evaluations is highly desirable. Ability to work collaboratively within a multidisciplinary team environment.
